Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our Primary Care - Sparta team. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.

Opportunity Details

• Position Type:Full-time, (0.8 to 1.0 FTE)
• Office hours:
  • Monday 8am to 5pm
  • Tuesday 8am to 5pm
  • Wednesday 7:30am to 5pm
  • Thursday 8am to 5:30pm
  • Friday 8am to 5pm 
• Location: 475 S State St Sparta, MI 49345
• Specialty:Primary Care, must be able to see all ages
• Team Size:3 APPs and 2 physicians. Office total 25.
• Call Requirements: APPs do not currently have call responsibilities
• Additional Details: Spanish speaking required

Requirements

• Open to new graduates
• Board certified prior to employment
• Active Michigan medical license (or ability to obtain)
• DEA license

Duties & Responsibilities

A typical day may include:

• Providing direct patient care
• Collaborating with multidisciplinary teams
• Participating in teaching and mentoring
• Documenting in Epic EMR
• Engaging in quality improvement initiatives
• Supporting community health efforts
