Description
Job Summary: As a Licensed Practical Nurse (LPN) in a Group Home you will assist learning disabled adults with physical/medical complications/conditions by assessing each individual’s physical health needs and providing person centered clinical care. Work in this role will help individuals overcome barriers and achieve a greater quality of life.

Occasional assistance in one of our other Group Homes may be required
Perform additional duties, or substitute for others as required. Additional duties may include but are not limited to, clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels, snow blowers while applying deicing agents to ensure safety
Supervisory Responsibilities:
Trains Direct Care Staff to complete medical procedures that they are allowed to perform as needed.
Essential Responsibilities:
Primary responsibilities include, but are not limited to:
providing nursing care
administering medication under the Medication Administration Plan (MAP) guidelines
overseeing and executing medical protocols
training and developing staff to execute medical protocols that staff are allowed to execute under MAP guidelines.
assisting individuals to become a more active part of their communities
communicate any change in status to appropriate individuals
perform individual assessments with any change of status.
provide skilled nursing interventions as indicated
develop positive relationships with LIFE residents, families, and staff
participate in day-to-day activities in the Home when no medical protocols or documentations required
complete required LIFE and DDS trainings as assigned.
other duties as assigned
Preferred Education and Experience:
LPN-Licensed Practical Nurse Credentials (REQUIRED)
High school diploma or equivalent (REQUIRED)
At least two years’ related experience required.
Must have current CPR/AED/First Aid certification or be willing to attend training through LIFE within 30 days of date of hire.
Must be willing to attend any LIFE or DDS required/recommended training classes.
Must have current Medication Administration Program (MAP) certification or be willing to obtain through LIFE as soon as possible.
Experience with individuals with intellectual disabilities preferred.
Experience with individuals with medical issues associated with aging.
Experience in a Group Home or Day Program ideal.
Requirements
Physical Requirements, Work Environment, and Travel:
Must be able to lift as needed manually and with the Hoyer Lift as needed
Must be able to safely transfer residents from chairs, beds, vehicles, etc.
Periods of sitting and working on a computer
Driving as needed to doctor appointments, errands, activities, and events
Able to be on your feet and moving for a large percentage of the day
Additional Requirements:
Able to pass all required LIFE pre-employment screening requirements:
Acceptable reference checks from previous employers
Criminal Offender Record Information (CORI)
Disabled Persons Protection Commission (DPPC)
Department of Developmental Services (DDS) Fingerprint-Based Criminal Background Check
Intellicorp Motor Vehicle Driving History - acceptable to LIFE’s insurance carrier
Reliable transportation, valid driver’s license and acceptable driving record required.
