Opportunity InformationSutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing General Cardiology program in San Francisco, California. This position works directly with General Cardiologists to support comprehensive cardiovascular care in an outpatient setting. The NP/PA will play a key role in managing established cardiology patients, improving access to specialty care, supporting care transitions, and providing patient education. This role is essential to expanding capacity for new patient referrals while ensuring continuity of care for patients with chronic cardiovascular conditions. This opportunity is ideal for an experienced Nurse Practitioner or Physician Assistant with a strong cardiology background who thrives in a collaborative, patient-centered environment and is committed to delivering high-quality cardiovascular care.

Opportunity Details:
- Conduct follow-up visits for established cardiology patients in the outpatient setting
- Support General Cardiologists by managing routine and chronic cardiovascular conditions
- Perform comprehensive cardiovascular assessments, order and interpret diagnostic studies, and develop treatment plans in collaboration with supervising physicians
- Support transitional care management through timely post-discharge visits and care coordination
- Provide education regarding cardiovascular conditions, treatment plans, medications, risk factor modification, and lifestyle changes
- Promote medication adherence and patient engagement to improve long-term cardiovascular outcomes
- Collaborate with cardiologists, nursing staff, primary care providers, and other members of the care team to ensure continuity of care
- Conduct in-person and telehealth visits as appropriate
- Manage patient communication and documentation through the Epic electronic health record
- Support quality improvement, patient safety, and patient experience initiatives
- Help reduce patient wait times and improve access to specialty cardiology care
- Work within a collaborative, team-based cardiovascular practice focused on exceptional patient outcomes
Clinical Focus: The NP/PA will support patients with a broad range of cardiovascular conditions, including:
- Coronary artery disease
- Heart failure
- Atrial fibrillation and other cardiac arrhythmias
- Hypertension and hyperlipidemia
- Valvular heart disease
- Stable chronic cardiovascular conditions requiring ongoing management and follow-up care
Requirements
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current California Nurse Practitioner or Physician Assistant license
- DEA Controlled Substance Registration Certificate (CSRC)
- Basic Life Support (BLS) certification
- Advanced Cardiac Life Support (ACLS) certification preferred
- Additional certifications as required by the department
- Prior cardiology experience preferred
- Experience managing cardiovascular patients in an outpatient, inpatient, or specialty practice setting strongly preferred
- Demonstrated ability to work effectively within a multidisciplinary clinical environment
- Strong communication, patient education, and care coordination skills
