Oregon Medical Group, part of the Optum family of businesses, is seeking an Internal Medicine, Geriatric Medicine, or Family Medicine Physician to join our Complex Care Clinic team in Springfield, OR. Optum is a clinician-led care organization that is changing the way clinicians work and live.This is an exciting opportunity to help shape a developing outpatient program dedicated to caring for older adults with complex medical needs. Located at Mohawk Medical Clinic, the Complex Care Clinic provides enhanced, team-based support for high-risk patients who need more intensive care coordination and medical management than traditional primary care can offer.
Whether patients are recovering from hospitalization, navigating multiple chronic conditions, managing dementia, or facing functional decline, you'll have the time, resources, and interdisciplinary support needed to make a meaningful impact on their health, independence, and quality of life.

Practice Medicine the Way You Envisioned
* Focus on medically complex older adults rather than high-volume primary care
* Longer appointment times allow for thoughtful, comprehensive care
* Manage a smaller patient panel
* Average of 16 patients per day
* Dedicated support for care coordination, medication management, and transitions of care
* Opportunity to develop expertise in complex chronic disease management, dementia care, and geriatric syndromes
Work-Life Balance

Position Highlights:
Clinical Care
* Provide comprehensive geriatric assessments across medical, functional, cognitive, psychological, and social domains
* Diagnose and manage complex, multi-morbid conditions commonly seen in older adults
* Lead care for patients with frailty, dementia, falls, mobility limitations, polypharmacy, and chronic disease burden
* Develop individualized care plans aligned with patient goals and values
* Conduct medication reconciliation and deprescribing initiatives focused on safety and improved outcomes
* Facilitate advance care planning and goals-of-care conversations
* Coordinate transitions of care following hospitalization, skilled nursing facility stays, and other acute care episodes
* Collaborate with caregivers and families to support long-term health and independence
Interdisciplinary Team Collaboration
* Collaborate closely with nurses, social workers, pharmacists, care managers, therapists, and behavioral health providers
* Participate in interdisciplinary case conferences and care planning meetings
* Provide clinical consultation and leadership to team members managing complex patients
Care Coordination & Systems-Based Practice
* Collaborate with primary care providers and specialists to reduce fragmentation of care
* Utilize population health tools to proactively manage high-risk patients
* Support efforts to reduce avoidable admissions and emergency department utilization
* Promote evidence-based approaches to complex geriatric care
* Help advance dementia care and innovative models for older adult health management

Required Qualifications:
* MD or DO degree
* Completion of an accredited Family Medicine or Internal Medicine residency
* Board Eligible or Board Certified in Family Medicine or Internal Medicine
* Oregon medical license and DEA registration, or ability to obtain prior to start
* Current BLS certification
Preferred Qualifications:
* Completion of an accredited Geriatrics fellowship
* EMR Proficient
* Excellent time management skills
Compensation for this specialty generally ranges from $237,500 - $384,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
